Medical Forms and |
• The State of CT requires physicals for all students entering pre-school and
Kindergarten and during the 6th grade year. Please return medical forms
for all PreK and Kindergarten students by August 15th.
• All students wanting to participate in basketball, cheerleading or track and
field must have a sports physical form completed with an exam dated after
June 1, 2011.

Are not available online. Look for hard copies in your Summer Packet envelopes. All math packets are due back the first day of school.
Suggestion: pace yourself and do a little bit every week. This will make it easier and keep your math skills strong all summer.
